Nominate an individual or organization/team who has positively impacted our lives through their contributions to both amateur and/or professional sports, achieved outstanding success, demonstrated exemplary values and personal characteristics, and made defining contributions to their sport. Nominees must live or have resided in Etobicoke or have had a significant impact on the Etobicoke community in the field of sports.

Nomination forms for the current year must be submitted by March 1st. Only completed forms will be considered by the Etobicoke Sports Hall of Fame’s Selection Committee. Please note that nomination forms are retained for a five-year cycle.
